Doctors in German hospitals are paid on published pay scales, so you can see exactly what a resident, specialist or senior physician earns before you apply. This guide uses the 2026 scale for municipal hospitals, explains what is deducted from gross pay, and shows how Indian doctors move up.
Headline figure: a resident doctor (Assistenzarzt) in a municipal hospital starts at about €5,722 gross a month in 2026, about €68,700 a year before allowances. A specialist (Facharzt) starts at about €7,552 a month.
Doctor pay scale in 2026 (municipal hospitals)
Most public hospitals run by cities and districts pay under the TV-Ärzte/VKA agreement. Pay rises with your grade (role) and your step (years of experience in that grade).

What is deducted from gross pay
Germany deducts income tax and social insurance at source. The employee's share of social insurance in 2026 is roughly:

Insurance
Employee share
Notes
Pension
9.3%
Total 18.6%, split with the employer
Unemployment
1.3%
Total 2.6%, split
Health
7.3% + half the insurer's additional contribution
General rate 14.6%, split; additional contribution about 2.9% on average
Income tax is progressive: the first €12,348 of taxable income in 2026 is tax-free (Federal Ministry of Finance), and the rate rises with income. Your tax class (single, married) changes the monthly deduction, so use a payslip calculator with your own details rather than a general percentage.
Pay before and after Approbation
Where you are in the licensing process decides which job and pay you can get:
Licensing stage and typical pay
Stage
What you can do
Typical pay
Learning German, no licence
Observe (hospitation), no clinical work
Usually unpaid
Berufserlaubnis (temporary licence)
Work as a doctor in one state, up to 2 years
Usually the resident grade if employed as a doctor
Approbation (full licence)
Work anywhere in Germany without limits; start specialist training
Hospital pay for licensed doctors is above the 2026 EU Blue Card threshold of €50,700 a year, so most doctors with a job offer qualify for the Blue Card (Federal Employment Agency). The Blue Card leads to permanent residence after 21–27 months, depending on your German level, and spouses don't need to prove German first.
University and private hospitals
University hospitals pay under TV-Ärzte/TdL, which has the same four grades but slightly different amounts. Private hospital groups negotiate their own agreements, often close to the public scales, and may add bonuses. When you compare offers, compare grade, step, weekly hours, on-call rules and specialist-training support, not only the base salary.
Example: a resident's pay over the first years
In grade I you move up roughly one step per year of experience as a doctor. Recognised experience from India can place you on a higher step from the start, so ask your employer to count it in writing.

From gross to net: what changes your take-home pay
Tax class: single people are in class I; married couples choose between IV/IV or III/V, which changes the monthly deduction (the annual tax return evens it out).
Children: they lower long-term care contributions and bring child benefit (Kindergeld), paid separately.
Health insurer: the additional contribution differs between statutory insurers.
Pension scheme: doctors in the medical pension scheme (Versorgungswerk) pay into it instead of the statutory pension.
Church tax: only if you are a registered church member.
On-call duty and allowances
On-call duty (Bereitschaftsdienst) is paid on top of the base salary, at rates set by the pay agreement and how busy the duty usually is. Night, Sunday and public-holiday work also carries supplements. For many residents these payments add a noticeable share to annual income, so ask how many on-call duties a month the department usually plans.
Does your specialty change your salary?
Not on the pay scale. A resident in surgery and a resident in psychiatry in the same hospital are paid the same grade and step. Differences come from on-call and shift work (emergency, anaesthesia and surgical departments usually have more), from private-patient pools shared by some senior doctors, and later from leadership roles such as senior physician or chief physician (Chefarzt), whose contracts are negotiated individually and are not part of the scale.

Specialist training (Weiterbildung): usually 5–6 years as a resident; becoming a Facharzt is the biggest single pay rise.
On-call and shift work: paid on top of the scale and a large part of many residents' income.
Region and employer: rural hospitals with shortages may offer extra benefits; private groups negotiate their own pay.
Moving up grades: senior physician and senior consultant roles bring the highest scale pay.
Cost of living to keep in mind
Rent is the biggest cost and varies most between cities: Munich and Frankfurt are far more expensive than Leipzig or the Ruhr area. Health insurance is deducted from your salary, so you don't pay it separately.
Frequently asked questions
What is the salary of a doctor in Germany per month?
In municipal hospitals in 2026, residents earn €5,722–€7,355 gross a month, specialists €7,552–€9,699, senior physicians €9,460–€10,811 and senior consultants from €11,128, before tax and allowances.
How much do Indian doctors earn in Germany?
The same as German doctors in the same grade: hospital pay scales do not depend on nationality. Once licensed, an Indian doctor starts at the resident rate of about €5,722 gross a month in a municipal hospital.
How much tax do doctors pay in Germany?
Employees pay social insurance of roughly 20% of gross pay (pension, unemployment, health and care) plus progressive income tax. The first €12,348 of taxable income is tax-free in 2026.
Do doctors get paid during the Berufserlaubnis?
Yes, if employed as a doctor: hospitals usually pay the resident grade. Observerships (hospitations) before you hold a licence are usually unpaid.
Do university hospitals pay the same?
Not exactly. University hospitals use the TV-Ärzte/TdL scale and private hospital groups have their own agreements, so compare the grade and step in each offer.
